Reporting to the Director Health Programs, this is the senior position of the Home and Community Care program for the region. This position provides program expertise and will coordinate the support to planning, program development, service delivery and evaluation for home and community care programs in the region. This will involve coordination of expertise across the department, in partner departments and agencies, and will emphasize community development and deepening of community capacity.

This position ensures the efficient and effective coordination and support of Home and Community Care programs in compliance with territorial standards of care. Home and Community Care ensures the delivery of quality case management, nursing and support services to clients in their home setting and other community settings, assisting them to function at their optimum level and to remain independent of institutional-based care services. This program provides nursing care and support services to clients throughout the life cycle that enable clients to stay in their homes and function at their optimum level while reducing demand on the acute care system.

The Manager Home and Community Care manages the Rehabilitation program and administrative operation in the Iqaluit region. Provides professional and clinical supervision to ensure the safe and effective delivery of Home and Community Care. Coordinates and facilitates the engagement of external parties with program expertise to support Home and Community Care Services. Ensures appropriate training is accessed by program staff. Facilitates program design to maximize community benefit .Ensures effective public accountability mechanisms are in place.

 Ability to work effectively in cross cultural setting ensuring that Inuit Societal Values are acknowledged and maintained during patient and employee interactions. Maintains knowledge of Inuit customs, history, cultural sensitiveness and local community characteristics. Ability to teach effectively on an individual or group basis. Ability to plan, coordinate, implement and evaluate a comprehensive home and community care services appropriate to Northern community needs. Leadership skills including the ability to inspire others and provide effective direction and delegation to team members. Supervisory and management skills to ensure the provision of quality services. Quality communication, negotiation and conflict management skills. Knowledge of Nunavut health and social service system. Basic computer skills are an asset.

 A Baccalaureate Degree in Nursing or equivalent required, with a minimum of 3-5 years community nursing experience. Minimum 3-4 years supervisory experienced required. RNANT/NU registration.  Basic CPR certification with annual re-certification is required. Wound care, diabetes care, palliative care, specialized foot care, peripheral intravenous (PIV) and central intravenous (IV) access certificates all an asset.

The Official Languages of Nunavut are Inuktitut, English and French.

 Knowledge of Inuit language, communities, culture, land and Inuit Qaujimajatuqangit is an asset.
