What do I do if my Identification card is lost/stolen or destroyed?

If your General Identification Card is lost, stolen or destroyed, please complete the form below and submit it to @email or your local Motor Vehicles Office. Individuals under 18 will require a signature from a parent or guardian. The parent or guardian must show two pieces of government-issued Identification.
